WASHINGTON — A new Government Accountability Office report released April 15, 2026, estimates that fraud in federal programs and operations costs taxpayers between $233 billion and $521 billion every year, with the upper end exceeding half a trillion dollars.

The report, titled “COMBATING FRAUD: Challenges in Managing Fraud Risks in Federally Funded, State-Administered Programs” (GAO-26-109093), states that all federal programs and operations are at risk of fraud, regardless of whether they provide financial or nonfinancial benefits.

Highest-risk areas include state-administered programs such as Medicaid, Unemployment Insurance, Temporary Assistance for Needy Families (TANF), SNAP, and WIC. In fiscal year 2025, the federal government sent an estimated $1.2 trillion in grants to state and local governments, creating oversight gaps due to decentralized delivery and limited data sharing.

GAO notes that fraud tactics have evolved with technology, including AI and stolen identities, and were amplified during the COVID-19 pandemic. The agency has issued more than 215 recommendations to improve fraud risk management since 2010, but about 40% remain open.

The report stresses that prevention is far more effective than “pay-and-chase” recovery efforts, which recover only pennies on the dollar. “These losses place an increased burden on the government’s financial outlook” and erode public trust, GAO warned.

For American taxpayers, the findings highlight the urgent need for stronger antifraud controls across all federal spending.
